VNA of Fox Valley receives $70,000 grant for the Aurora Community Heart Clinic (Feb 2011)

Aurora Community Heart Clinic

The Aurora Community Heart Clinic (ACHC), a specialty clinic for underinsured and uninsured adults, 18 years and older, was created to provide access to affordable cardiovascular care in the Aurora area.

Cardiovascular disease which includes coronary artery disease, stroke, peripheral vascular disease, uncontrolled blood pressure, and heart failure is the leading cause of death both locally and nationally.

Patients are seen at the Visiting Nurse Association (VNA) of Fox Valley facility at 400 Highland Avenue in Aurora, Illinois. Patients coming to the clinic must be referred from their primary care provider for a cardiovascular related problem requiring specialized cardiology care. Qualifying patients will be seen by a cardiovascular trained mid-level provider (nurse practitioner or physician assistant) under the supervision of a board certified cardiologist.

The goals of ACHC include: provide access to specialized care; improve patient understanding of disease process and personal risk factors; improve patient outcomes including blood pressure and cholesterol control; achieve high levels of clinically appropriate treatment; and provide education for life-style modification to decrease risk.

ACHC accepts Medicaid and sliding fee scale payments for those individuals that are uninsured and low-income. Sliding fee scale payments are determined based on household income and family size.
